Parents must complete the following enrollment forms
before the first day of attendance:

A physician must complete and sign the forms listed below.
They must be completed and in Child Garden's possession prior to admission.
*Healthcare Summary- must be renewed
every year for children under 24 months of age. If over 24 months, must be renewed each time the child advances to the next
age group.
*Immunization History- a signed, notarized statement of parental objection to immunizations,
or a medical exemption form must be on file at the time of enrollment when, for any reason, all immunizations are not current.

Payment policies
Tuition must be paid in advance. Please follow one of
these payment plans:
Monthly - Due 1st of each month
Bi-weekly - Due 1st
Monday beginning the two weeks
Weekly - Due each Monday

Child Garden will be closed on the following holidays
(or observed holidays) each calendar year. See current school calendar for specific dates.

Nap and Rest Times
Child Garden has a rest time each day from 1:00 - 3:00
p.m.
Hours of operation
The hours of operation for Child Garden are 7:00 a.m.
to 6:00 p.m., Monday through Friday. Please refer to the current school calendar for days that the schools are closed.
Child Garden only has a full time option. You are
able to drop off and pick up your child within the hours of operation.
Field Trips
Child Garden believes that children benefit from visits
to relevant off-site locations. Some examples of field trips include parks, zoos, museums, walks, swimming, and plays. Written
permission from parents is required before all field trips. In the summer, Child Garden collects an activity fee that will
cover all field trips and activities for the summer months. With the exception of walks, transportation for field trips is
provided by a professional transportation service.
Parents are always welcome to volunteer to accompany
their child's classroom on field trips. Parents of children aged two and younger must accompany their child if they wish for
them to participate. Additional fees may be incurred for these trips.
Meals and Snacks
Meals and snacks are included in the tuition. Lunch menus
are available by the first of the month. Child Garden does have an on-site chef who cooks the lunches. Snack foods are provided
by Child Garden. Parents are encouraged to bring a fruit or vegetable in on Mondays for their child to share with their class.
Illness Exclusion
Your child must be free from any of the following symptoms
for 24 hours before s/he may return to school:
*If child has a temperature of 100 degrees Fahrenheit,
axillary (under the arm), or higher before fever reducing medication is given.
*If child has a sore throat and has been
diagnosed with Strep. Your child has to have been on medication for 24 hours (not just 3 doses) before returning to school.
*If
your child has been vomiting. S/he must be absolutely free from vomiting for 24 hours before returning to school.
*If your
child has a severe cold or respiratory distress.
*If your child has diarrhea. S/he must be absolutely free from diarrhea
for 24 hours before returning to school.
*If your child has conjunctivitis (red, water, crusted, or pus draining from the
eye). S/he must have been on medication for 24 hours before returning to school.
*If your child has an undiagnosed rash
or a rash attributable to a contagious illness or condition.
*If your child is unable to participate in regular program
activities and requires more care than can be provided by our staff without compromising the health and safety of the other
children.
Please notify the school within 24 hours if your child
is absent with a communicable disease so that we can post a notice for other parents.
We are only allowed to give prescriptions or over the
counter when it has been prescribed by a doctor. The medication must be in the original prescription bottle (the pharmacist
will give you an extra bottle so that you may bring in only the amount needed at school) and be clearly marked. You must also
fill out a medication form which is available from your child's teacher.
This policy is based on recommendation from the Department
of Human Services and the Minneapolis Health Department. This policy is very important and must be strictly enforced for the
welfare of all the children in our care.
Your help in screening your child for any of these symptoms
and keeping him/her home if there is any doubt in your mind as to whether s/he is feeling well will be greatly appreciated.
Severe Weather Closing
Child Garden will always make an effort to remain open
during severe weather. In case of severe weather, the Director, General Manager, and Owner will determine if the center will
be closed all day, open late, or close early. Listen to WCCO 830 AM or watch WCCO Channel 4 for the "Child Garden Montessori"
listing.
If it becomes necessary to close the center due to severe
weather after children have arrived, a staff member will notify parent(s) immediately. The parent or authorized person will
be requested to pick up the child as soon as possible, but no later than the closing time decided upon by the school.
Tuition will be due for any days the center is closed
for severe weather, as it is Child Garden's policy to continue to pay for staff and expenses for running the center.
